Evolution VIP Membership

We created Evolution VIP Memberships because of our commitment to design customized, results-driven treatment plans that will help transform our patient’s outer radiance and inner confidence.

Welcome! By signing up for our monthly membership, you agree to the following terms and conditions. Please read them carefully.

 

Membership Overview: 

 

By subscribing to the Evolution VIP Monthly Membership you will receive access to exclusive pricing and services on a recurring monthly basis. Your subscription will automatically renew each month unless canceled.

 

1. Billing & Payments

  • Memberships are billed monthly, beginning on the date of sign up

  • All payments are processed securely

  • A 3 month minimum is required

  • You agree to provide valid, up-to-date payment information at the time of registration and keep it updated throughout your membership

  • Membership cannot be combined with other discounts/special pricing

  • 10 units of toxin offer: First redemption 90 days from sign-up date and every 90 days thereafter.  Each redemption can be done within 2 weeks after the 90-day mark.

2. Renewal & Cancellation

  • Your membership renews automatically each month

  • After 3 months, you may cancel your membership at any time

  • Cancellations must be made at least 30 days before the next billing date to avoid being charged for the next cycle

  • No partial refunds will be issued for canceled subscriptions

3. Refund Policy

All membership payments are non-refundable. If you believe you were charged in error, please contact our support team for review.

 

4. Changes to Pricing or Terms

We reserve the right to change membership pricing or features at any time. You will be notified of any changes at least 30 days in advance. Continued use of the membership after changes go into effect constitutes acceptance.

 

5. Account Access & Security

You are responsible for maintaining the confidentiality of your account information. Sharing your membership access with others is strictly prohibited and may result in termination without refund. Membership is non-transferable.

 

6. Termination

We reserve the right to terminate or suspend your membership at our discretion, especially in cases of violation of these terms or misuse of the services.
